ConnPACE: Helps eligible senior citizens and people with disabilities afford the cost of most prescription medicines. If you are a Connecticut resident aged 65 or older, or with a disability aged 18 or older, you may qualify for ConnPACE. Besides covering most prescriptions, ConnPACE pays for insulin and insulin syringes and needles. Income limits are: $25,100 for single persons and $33,800 for married couples.
There is an annual ConnPACE open enrollment period from November 15 to December 31 of each year with eligibility beginning on January 1.
- The ConnPACE open enrollment period coincides with the annual open enrollment period for the Medicare Part D Program and is for NEW applicants only.
- New ConnPACE applications will not be accepted outside of the open enrollment period unless the applicant is within 31 days of his/her 65th birthday or becoming eligible for Social Security Disability or Supplemental Security Income.
- Again, the ConnPACE open enrollment does not apply to current ConnPACE members. Individuals who are currently eligible for ConnPACE will continue to renew their benefits in the month their renewals are due.

Extra Help with Prescriptions: If you are receiving Medicare and have limited income and assets, you may qualify for extra help with prescription and drug coverage. The amount of help you receive will depend on your income and assets. Help may be available to pay premiums and deductibles. Income and asset guidelines are:
Family of 1: Income less than $14,700, Assets less than $11,500
Family of 2: Income less than $19,800, Assets less than $23,00
Elderly and Disabled Renters’ Relief Program: Applications will be available from May 15 through September 15 in the Social Service Office by appointment only. Proof of residency and income are required. SSA Form 1099 and a 2008 income tax return (if applicable) must be provided. Income limited for the State of Connecticut program are: Single: $29,800 and Married: $36,500. Please call the Social Service office at 860-844-5351 for additional information and applications.
Granby Food Bank/Farmington Valley VNA
This community service is offered by the VNA to any family or individual who may need assistance in meeting daily living expenses, particularly due to the loss of a job or Medicaid/Medicare limitations. Please contact the Social Service office for eligibility screening and registration.

Meals on Wheels: McLean Home provides meals for ill, homebound, or disabled persons, delivered by volunteers five days per week. Sliding fee scale is available. Call 860-658-3700 for additional information.
Salvation Army
Provides emergency assistance and disaster services to the needy. Programs include financial assistance, emergency shelter, outreach through home visits, and referral and advocacy for services. Contact Social Services at 860-844-5351.

Transportation Activity Program (TAP): 860-844-8859
Volunteer drivers provide free transportation to medical appointments in Granby and surrounding towns for those people who do not have any means of transportation. Call for ride appointment one week in advance if possible.
